INTRODUCTION |
|
Passport Office, Surat was inaugurated
as on 16.08.2003 by Shri Yashwant
Sinha, then Hon'ble Union Minister for External Affairs. The
chief guest was Shri Kashiram Rana, Chief Hon'ble Minister
for Rural Development. It caters
8 (eight)
districts viz Surat City/Dist, Bharuch, Valsad, Navsari, Dang,
Narmada, Tapi/Vyara, Bardoli. The office is located at A.P.M.C.
Building, 2nd floor, New Sardar Market, Puna-Kumbhariya
Road., At /Po- Dhumbhal, Surat 395010. |

Applicant must approach personally with a written request letter.
While meeting to Passport Officer, please
bring the relevant documents e.g.
VC as per Annexure-F (two copies in original) on official stationery (letter head) with
photocopy of Identity of verifying Officer & Annexure-I alongwith other requisite documents. |

DELIVERY OF
PASSPORT |
| All Passport are sent by SPEED POST
only and except in special case/emergency (promised by Passport
Officer) . Delivery of Passport will not be given on an authority
letter. |
IMPORTANT NOTES |
|
1) On-line registration is mandatory at
Passport Office, Surat (exception for miscellaneous services i.e.
PCC/ECNR)
2) Payment for online submission for fresh
application/misc. services/Tatkal Charges/Penalty charges are
accepted in cash.
3) Hand written applications and
on-line application can be submitted at the DPACCs and Speed Post
Centers also alongwith valid DD.
4) All the requisite
original documents should be produced for verification and
photocopies of documents should be self attested.
5)
All columns should be filled in or if they are not applicable
that should be specifically marked as NO, Yes or Not
applicable.
6) In case the passport is issued prior to
marriage and applying for re-issue, marriage certificate is
must.
7) In case of all married females applying for
the first time, the application should be supported by marriage
certificate or joint marriage affidavit with joint wedding time photograph duly
attested by Notary.
8) In case of minors those who are
born on or after 26.01.1989, birth certificate is must.
9)
Illiterate person above 45 years can submit notarized affidavit
indicating place and date of birth in addition to Election Card or
Pan card.
10) Case of minor is to be submitted by
father/mother only. If both parents are abroad, guardianship
affidavit and 2 witnesses affidavits known to guardian, minor and
parents should be submitted. This should be accompanied by consent
letter from parent authorizing the guardian duly authenticated by
Indian Mission abroad.
11) When applicant is holding a
valid passport and suppressing changes the personal particulars,
criminal case is pending against him/her and this information is not
disclosed in the application. They are liable to be prosecuted under
Section 12(1)(b) of Passport Act, 1967 registering criminal case
against them.
12) No enquiry either in
person or on phone shall be entertained before 45 days
on submission of Passport application.
13)
In case of Central Govt./ State Govt./ PSU/ Statutory Bodies, employees should
furnish Identity Certificate (NOC) as per Annexure-B (two copies in original) & alongwith Annexture-I.
Please NOte that routine pattern NOC shall be rejected.
